Hi there, so sorry you've having problems.

I think it is very difficult to help in any way, because we don't know much else about your koi.

Where are they kept, a tank or pond? How is it filtered? Has it been running long? What are all your water parameters?

These help us to understand what has been an accumulation to lead your koi to be like this.

To me it looks like an injury which has become infected. This could be due to many things, most likely a water issue.

I hope you don't lose him, none of us like losing any pet.

hi if you could come back with the parameters reading would be great it looks as dawn has said an secondary infection has set and has started to form an ulcer.

once we find the cause of the issue we will advise on treatment there is no point untill then advising you to use this or that as it might just be wasting your money and time.

we have him in a tank, he was in a big tank with the others but i moved him in to a smaller one with another goldfish i have that seems to have growths but it has had them for years and none of the treatment has helped him, the big tank has been running for years with water changes done every so often, we try to keep a good amount of the water they are use to, it has 2 fulval 4s one old one and the other a new one, the small tank he is now in is newly set up but we took must of the water for it from his old one and topped it up with fresh stuff, the fulval in there is also a 4, the ph has been around 7.5 to 8, the ammonia levels are negative, the small tank is around a 20" by 10" by 13", all of my other koi, goldfish and tench look healthy and active, they remain in the bigger tank, i am sure it was an injury as it started small like a red dent that he may have banged it on a stone or something but it has just gradually getting worse/receding, i clean the sponge's out every 3-4 weeks. hope this all helps? thanx for helping, hope we can help him!

Unfortunately, koi aren't meant for tanks long term. They grow much to quickly and produce a lot more waste than other fish resulting in a water imbalance and then poss injuries through flashing off objects.

I can only hope that something will work and help him. But if you say the water is fine, then things should improve. Is it heated at all? That will also help in their healing time.

I'd try a much larger body of water, a few hundred gallons. ~Heated, but you might not have that sort of space.

Keep us posted, sorry I can't be of any further use.

i have notice the waste, no there is no heater in the big tank, but i should be able to get one, the small tank has one but not sure how it is working, what kind of temp would u recommend? it was a short term thing, but money is a little tight just now and with winter not sure it would be good for them outside just now, hopefully we can get a pond running in the new year, i am thankful for all of your help, he seems to be a fighter, ta x

right before we start about the temp you need to be sure you are testing the water in the small tank this is key as if you are raising the temp the ammi level etc will become more toxic .

what is against you is the volume of water i est you have less than 9 gallons which will be hard to keep correct daily water changes will be needed.

then sorry if it sounds like im going on abit you can slow 1 deg per day raise the temp to 23-24 degs and buy yourself an ulcer treatment kit this will have full instructions on how to use and within no time you should see an improvement in the fish.

thou we are here if you need any advice ;)
